The Pine Hills Wellness project seeks to invest in one of Orange County’s neediest communities to increase awareness of and access to healthcare services through navigation and community outreach activities. The initial project was designed to serve children ages 2 – 7 years but has since expanded to serve the entire Pine Hills community. Our community wellness coaches help families understand their benefits and assist families in building strong relationships with their primary care physicians while reducing unnecessary trips to the emergency room. The project has been expanded through a grant from The Center for Medicare and Medicaid Services (CMS) to partner with the Evans Wellness Cottage to serve students and staff at Evans High School as well as the greater Pine Hills community. The Cottage offers both medical and dental services. These health care services coupled with health education programs and other community based services through the Evans Community School offer students complete access to all of their health related needs.
CHS has also partnered with the National Association of County and City Health Officials (NACCHO) to offer breastfeeding and lactation education, support, and resources within the Pine Hills community. The program serves new and expecting mothers and connects them to resources such as a website designed specifically to offer education and support and allows them to interact with their wellness coach.
